Upon completing each "mode", you will unlock the next one, with Nightmare being unlocked after Normal, then Hell, and finally Inferno. Most of the bosses in this game are skippable if you join a coop game that has already progressed passed the associated boss, but for leveling purposes, it is recommended that you stay as close to the story as possible for two reasons: leveling allows you to be better prepared for each new Act in the game, as each of the 4 acts is harder than the first, and also the higher modes (Nightmare, Hell, Inferno) require you to attain a certain player leveling prior to accessing them. If you're new to the series, you will probably want to continue honing your skills in the non-Hardcore portion of the game prior to doing the Hardcore achievement. You will need to get thirty levels in Hardcore mode for Demon Slayer (Hardcore), which may sound quite difficult, but thankfully you can still set the game difficulty to Easy. Once you unlock Rite of Passage and reach level 10, Hardcore mode will unlock and you will be able to start a new character and press during character creation to make put them on Hardcore mode, where death is permanent. Rite of Passage, Demon Slayer and Hero of Sanctuary will unlock as you reach levels 10, 30 and 60, respectively, in non-Hardcore mode. You will also need to reach level 60 on at least one character in the Normal game, and level 30 on one character in Hardcore mode. Achievements aside, this is a very enjoyable game with enough variety in classes, and enjoyment from killing demons and getting loot, to keep pretty much every player satisfy even beyond the many hours it will take to 100% this.
